Altan is 11 years old and was admitted to the hospital for agitation, grandiosity and distractibility. This has been going on for one week, as reported by his mother. He hasn’t been sleeping, and on two evenings he took apart his room to “reorganize it” becaue he believes he is going to be a “famous YouTuber” for his organizational skills”. He has been emailing and texting family members to “leave the keys to their homes for him so he can organize their belongings while they are at work”. He get angry when his mother tries to reason with him, which is followed by him becoming excited and reassuring his mother that he is going to be “so wealthy from his YouTube channel that she won’t have to worry about him at all in the future”. These symptoms rendered Altan unable to pay proper attention to his bodily grooming habits, such as showering and changing clothes, or attend school. During the MSE, Altan was hyperverbal, unable to remain seated, displayed an irritable and labile affect, and flight of ideas. Altan does not have a history that includes these symptoms; however, his mother reported Altan was depressed, sleeping a great deal and lost weight a month prior.
A physical examination, including an MRI yielded unremarkable results.
