Positive information
Positive information on the patient includes nightmares that occur twice a week and wake her up screamy, bewildered, and clammy around 11 p.m., when she is most vulnerable. They cannot calm her down and she claims she does not remember all of these incidents. Due to her breech presentation, she was delivered by the caesarian section at 39 weeks of pregnancy. When she was a baby, she started full-day kindergarten, and her father vapes at home. Her mother and father are both social drinkers on the weekends. The gingivae of her mouth are swollen and erythematous with dental caries B, C, M, and L on them.
Negative information
She said she likes school and plays soccer twice a week in a 30-minute game league. Her pregnancy was trouble-free, and her delivery was trouble-free as well. Nobody in her family has ever been diagnosed with a chronic disease. She also has no problems with her respiratory system or cardiovascular system. She has no musculoskeletal problems. She has no neurological problems.
Would there be any other information you would want to obtain?
Among the details I’d want to gather from the patient is whether or not she consumes a lot of sweets. Describe a typical day in the life of this woman’s diet. What are her dental hygiene habits, such as how many times a day she brushes and flosses?
Differential diagnosis list with at least 3 possibly actual diagnoses
Sleep terror- To put it another way, according to Leung and colleagues (2020), sleep terror is an episode of screams and writhing in your sleep that occurs 2-3 hours after you’ve fallen asleep.
Hypnagogic hallucination- According to August and Maski (2020), hypnagogic hallucinations are transient but sometimes extended sensory experiences that occur during the transition from a hypnagogic (awake to sleep) or hypnopompic state (between sleep and wakefulness) (sleep to awake).
Nighttime panic episodes, also known as nocturnal panic disorder, occur when a person wakes up from sleep with somatic symptoms such as perspiration, palpitations, and chest aches, yet there is no apparent reason (Ekambaram et al., 2021).
Create a plan utilizing clinical practice guidelines for the priority diagnosis.
Sleep terror is self-limiting, and hence parents need comfort, the establishment of environmental safety, and instruction to help their children overcome it. Before the attack occurs (about 11 PM), the kid should be awakened and kept awake for roughly 4-5 minutes, following which they can go back to sleep. Gigliotti et al. (2021) state that a kid should be prevented from injuring themselves by eliminating potentially harmful things from the sleeping space and providing a clear departure route.
Additionally, because the patient’s mother is an obese diabetic, a nutritionist will recommend that she seek nutritional counseling to enhance the nutritional status of her kid. The child’s weight loss will be aided if the parent encourages the youngster to participate in regular physical activity. When the kid was diagnosed with dental caries, he or she will be instructed to restrict the child’s intake of sugary foods and beverages and to visit the dentist regularly.
